- Psychiatry Follow-Ups (established patients only) $105 (Same-Day Cash Pay Price)
- Psychiatry extended (more than 40 minutes, established patients only) $135 (Same-Day Cash Pay Price)
- ADHD or cognitive testing $100 (Same-Day Cash Pay Price)
- Personal Injury Cases will be billed at regular pricing unless paid on day of service
- Forms- Disability, FMLA, etc. $25
- Forms needed in less than 48hrs $50.
